Inquiry members
The Government Inquiry is chaired by Sir Mark O’Regan, a retired Supreme Court Judge and former President of the Court of Appeal.
He is supported by:
- Dr Helen Anderson QSO, former Chief Science Adviser and Ministry of Research, Science and Technology CEO.
- Steve Symon, a highly regarded New Zealand barrister with extensive expertise in regulatory enforcement and criminal law.

Secretariat and counsel assist:
- Helen Wyn, Executive Director leading the secretariat, a senior public sector leader with inquiry and public policy expertise
- Tom Powell, Counsel Assist, previously led investigations at the Infected Blood Inquiry (UK) and the Royal Commission of Inquiry into Abuse in Care (NZ).
- Karen Jones, Counsel Assist, with experience in investigations and litigation on serious and sensitive matters, including coronial inquests and the Dilworth Independent Inquiry.
