Use form and attach spreadsheet of claims to email to DIA on the first week of each month.
4. Adjustment request form
Application to the Secretary for Local Government by a territorial authority for adjustment (PDF, 124KB)
Use form but if more than one adjustment required attach a spreadsheet to the email. Only send one adjustment request per day.
