Application Forms and Fees
Application forms on this website are in Adobe Acrobat (.pdf). You will need to have Adobe Acrobat Reader 6 or higher installed on your computer to view or download the form. You can download this from the Adobe site. Once you have clicked on the link we suggest that you select ‘Open’ to view and print the application form rather than ‘Save’ as our forms change from time to time. Once you have accessed and printed the form, complete the necessary information in pen and send the required supporting documents to the Passport Office. Please note that application forms must be handwritten. The Passport Office will not process forms completed on a computer. You can also order application forms from the Passport Office. All payments must be in the currency of the country to which you are sending your application for processing. Please do not send cash through the mail. If you are sending your application to New Zealand from overseas, payment can also be made by bank draft or bank cheque made out in New Zealand dollars drawn on a New Zealand bank. Payment can be made by Visa, Mastercard, American Express, Switch, Solo, cheque or money order made out in Great British Pounds Sterling. Make cheques and money orders payable to 'Department of Internal Affairs'. Payment by personal cheque will not be accepted for either the urgent or callout services. General Fee Information Urgent Service Fee The urgent fee can be waived or reimbursed when written evidence on official letterhead from a doctor, hospital or police is produced showing that the need to travel is due to bereavement or serious illness. Callout Service Fee This fee is only charged once where multiple members of the same family or group are applying at the same time, however the urgent fee applies for each application for the rest of the group. For example, if a group of three people requires a callout service then one "callout service" fee would be charged and two "urgent" fees would be charged. As with the urgent fee, the callout fee can be waived or reimbursed when written evidence on official letterhead from a doctor, hospital or police is produced showing that the need to travel is due to bereavement or serious illness. Handling Fee for New Zealand Diplomatic Posts If you wish to send your passport application(s) to New Zealand, Sydney or London via a New Zealand Ministry of Foreign Affairs and Trade (MFAT) Post, a handling fee of NZ$50.00 is required. The handling fee is in addition to the relevant passport application fee. It also covers the return of the passport(s) via the Diplomatic Bag to the Post. Only one handling fee is required per family/group. Payments to Diplomatic Posts If you are submitting your application through a New Zealand MFAT Post, payment can be made by bank draft or bank cheque, which should be made payable to the Post concerned in local currency. You can also use cash if submitting the application in person (do not send cash through the mail). Dishonoured cheques If a cheque is dishonoured by the bank, the applicant will be liable for any costs incurred by the Department of Internal Affairs in recovering that fee. Additional costs may include any reasonable bank and debt collector charges. Where fees have not been paid in respect of an issued passport, the validity of the passport may be affected. No further passports will be issued until outstanding debts have been paid. |
